Full Consultation $150
A consultation includes a two hour conversation about the specific needs of you or your loved one. This includes a home visit, listening & addressing concerns, recommendations on keeping the home environment comfortable, advanced care planning, local and community services available, patient advocacy, education about planning for the end of life, physiological death education, mapping support for the care circle, final resting place options and help with facilitating family discussions. Discussions can also include care needs, explaining medical issues and creating a personal support plan. This is a personal customized session meant to address the specific individual needs at the end of life, offer collaboration, resources available and guidance.
Death Doula Services Starting at $50/hour
This includes many items listed in the consultation and also holistic emotional support, end of life planning , evaluations of needs, calm & compassionate presence, education regarding the physical signs of dying and what to expect. Options of dying, education regarding end of life comfort medications, non-medical comfort measures. Planning bedside vigils, the environment, sights, smells, lighting, noise levels, sensory preferences for the desires of the individual.
Respite Care Prices Vary
This service is offered to provide relief for primary care givers during one of the most difficult times of life. Sometimes end of life events coincide with other family events. Or maybe some much needed self-care is in order. The hourly rate is $50/ hour, overnights start at $600/night.. This does not include the gas fee, which applies for all travel over 30-miles.
Life Review Package $450
The life review package will help you or your loved one integrate life experiences into a visual art piece, memory book, scrapbook, note book, college, voice recording or final document. This session includes a 3 hour story telling session where information is collected and documented. The session will also include a discussion on how best to preserve the information, write the memories and who to share it with. The creating of a final version of the document or memory piece will be created by Autumn with the help of Timber G.. A 1-2 hour follow-up retrospection will be done with you or your loved one. This time will be to review and finish the project. These sessions can be broken up into shorter sessions to accommodate for fatigue. The finished piece is yours to keep.
Senior Pedi- Nail Care $50 Finger Nail Care Add on $20
Nail care for elders is important and can prevent serious complications. All nail care is provided by an RN. A nail care appointment includes a comprehensive assessment, plan of care, safe nail trimming and debridement (using sterile equipment), callus and corn management, specialized diabetic care, skin treatment, cleansing and gentle moisturizing. If there are concerns these will be discussed and a plan will be made in collaboration with the PCP. Keeping feet cozy and kept.
Medical Care Navigation $40
Support in understanding medical terms, preparing questions for providers, and navigating care decisions. This service incudes explaining medical jargen, explaining procedures and support care decisions. This does not including giving medical advice, or any medical opinion.
